 Cost:                          $20.00 Cash or Check (Must be signed up and paid prior to attending)

*If you are not signed up, you will not receive a physical, and you cannot sign up there*

 Who:                          All athletes (7-12), All fine arts (7-12), ROTC and HOSA students in GISD

 What to Bring:        - The UIL medical history form (revised 1-6-09) must be filled out                   completely and signed by a parent/guardian before you will be allowed to receive a physical.

                                    - The UIL physical examination form to be filled out by the physician.

     *This is the only form they will complete (No Summer Camp Forms)

- T-shirt, shorts and tennis shoes

- Packets (Optional but recommended):

We will be collecting completed packets only at the physicals.

Catastrophic Insurance ($10 – football, $5 – for all other sports) must also be paid at the time you submit a completed packet. The complete physical packet includes approximately 12 pages.

 Time:                       8:00am (GHS, Benold, Forbes)

Once the last GISD student in line goes through the door, they will move on to the next school. If you are not in line, you will not receive a physical later that day and there will be no refunds. Other schools have physicals after we are done so please be on time.

Copies:                     If you would like a copy of your physical (for all participants), please pick it up in the GHS Athletic Training Room from May 22 – May 24. This is especially important for those students who may be attending summer camps. After that, you will have to wait until August 6 to get a copy of your physical.

Sign-up:                   Middle Schools: Take $20 to your Athletic Coordinator by May 4, 2012

                                    GHS: Take $20 to your Athletic Trainer by May 4, 2012
